Multiple Choices IF/THEN

Hello,

I'm looking for a possible IF/AND/THEN help with a formula for 15 choices in cell D3 and 4 choices in cell E4 and a date must be entered in C3, then in cell F3, show the result of the date in C3.

Example:
If on 10/1/18 (cell C3), someone came in and "Applied" (cell E3), for "OJT" (cell D3), then a 10 for October would be entered in cell F3 (but instead of the only choice being "OJT", there are 15 choices for cell D3, and instead of the only choice being "applying", there are 4 choices in cell E3). if no choice in both cells D3 and E3, then nothing in F3.

I keep getting a security warning when I try to look at a response, it says: Sucuri Website firewall Access Denied. Is this tied to this forum at all or am I being redirected to unauthorized site?
