If you create a macro in Outlook 2010, then go to File-->Outlook Options-->Customer Ribbon, and change the "Choose Command From" to "Macros", then you should be able to add a button to a tab.
Thanks Ken, I guess I should have been more clear, is there no way to add a button via code except visual studio? If I customize the ribbon manually can I change the text dynamically?
