Using checklist to populate datasheet.

The project I'm working on has a checklist that multiple users will be able to access, that needs to track the data for various forms and graphs. 35 questions done five days a week. I was hoping to be able to use a simple true/false check box for the questions and them a submit button to dump that into a row on another sheet.

Is this possible, and how do I do it if it is?

With VBA anything is possible. The attachment is only one way of many I'm sure.

Hope you can adapt something to your needs.
