hyperlink mailto address points to the wrong cell

I have a column with names and another column with email address and use hyperlink on the email address.
I have a large number of rows of names & email addresses.
The "text to display" shows the correct information - however, the email address "mailto" shows the email address of the user a few rows below.
I am not sure what caused this problem and how long it's been like this.
How can I :
1. correct the problem
2. ensure it doesn't happen again.
